Original 1st printing 16&3/4 x 10&3/4" mailed (with cancelled 8/27/1969 stamp) bi-fold order form for the first series of 1969 "Moon Posters" by Peter Max.

 Art by Peter Max, medium-thin flat vellum stock mailer is in very good to excellent (B/B+ to B+) condition; there are horizontal/vertical factory folds (as issued, folded into 1/6ths), along with the aforementioned cancelled stamp and address label (item was sent to the City College Bookstore in NYC), a tiny edge ding or two and a few VERY light/soft surface scuffs here and there, otherwise fine.

 Nifty little mid-'69 oddball Peter Max artifact, I couldn't find any references to this piece ANYWHERE... VERY uncommon!
