Original 1st printing 3&5/8 x 2&1/4" double-sided fan-club membership card for Jefferson Airplane (featuring original drummer (and later founder of Moby Grape) Skip Spence).

 Designer unknown, medium-thick flat stock card is in excellent (B+) condition; there is a nearly-invisible tape shadow and a tiny corner-tip bend in the lower left corner, otherwise it is a fairly sharp example of this uncommon 1966 Jefferson Airplane artifact.

 I have seen flyers, handbills and posters for the "J.A. Loves You" fan club, but this is the first membership card I have EVER seen.
